The Coordinated Veterans’ Care (CVC) Program
Australian General Practice Network (AGPN) in conjunction with the Australian Practice Nurses Association (APNA), Flinders University, Baker IDI Heart and Diabetes Research Institute, have been engaged by the Department of Veterans’ Affairs (DVA) to provide training and resources in chronic disease management for general practitioners (GPs), practice nurses and community nurses who will be implementing DVA’s new Coordinated Veterans’ Care Program.

The Australian General Practice Network's role is to provide training in Chronic Disease Management to GPs and nurses implementing the program, as well as producing a range of online program resources such as templates and guidelines.
CVC training modules are now available online at www.cvcprogram.net.au/training

Module One - Coordinated Veterans' Care : Is your service ready? is now available.
Module Two - Care Planning and Coordination with the Flinders Program TM is now available.
Module Three - Managing Care Plans with disease-specific elements is now available.
The training and resources for GPs, practice nurses and community nurses will progressively be made available on the CVC Program website www.cvcprogram.net.au . Training will be provided in four modules. All modules will be available online and in CD/DVD format. An optional workshop to supplement the online learning for Module 2 will be held in all capital cities and in some regional areas.
